Community Walks:

                Enjoy some fresh spring air as you get your body moving in a short round trip walk with friends, family, or neighbors. All walks will begin at the location noted above. Please remember to wear light colored or reflective clothing and appropriate walking shoes. This is not a race so you can complete at your own pace. Please note that walks will be cancelled if the temperature is below 30 degrees or there is moderate precipitation.

Screening of The Weight of the Nation:

                Join HCPHNS staff in a screening of a portion of the HBO documentary film. Insightful and telling documentary about the obesity epidemic in The United States. Join us for all or a portion! Head home with some healthy recipes to try and a better knowledge of how you can help America win by losing (pounds)!

  Community Food Tasting and Sampling:

                Come sample and take home some yummy goodies prepared by the staff at HCPHNS. Sample healthy brownies or try one of the many healthy spins on old favorites! Go home with new recipes too!

 Mommy and Me Exercise Class:

                Join our staff Physical Therapist, Erica Mahoney, for a ½ hour long class designed for moms with children under a year.  Learn how to develop health exercise habits for you and your little one.  Bring your baby and get in shape!

Seniors Exercise Class:

                Physical Therapist, Erica Mahoney, will help seniors learn how to do safe and effective strength training exercise in their own homes. Use common household items or simple routine moves to strengthen your core and body for a healthy you!

 Stretching/Yoga Class:

                Stretch. Ever feel like you aren’t as flexible as you once were? Join Erica Mahoney, Physical Therapist, for a class on stretching and staying flexible to keep your body limber and pain free.

 

 Announcing the First Annual Spring Photo Contest:

HCPHNS is holding its first Annual Spring Photo Contest!

Submit your favorite spring photo/s - taken by you - to HCPHNS by April 22, 2014.

Photos will be judged by HCPHNS staff. Prizes will be awared for 1st, 2nd, and 3rd places. The winning photos will also be featured on our website, Facebook page, and in various newsletters for the month of May!

 Contracted Medical Social Worker

  • Instruct, treat, observe and evaluate patients exhibiting significant social and emotional problems affecting their health status.

  • Participate in the development and periodic re-evaluation of the physicians Plan of Treatment.

  • Instruct and counsel patients and families in treating and coping with social and emotional response connected with the illness.

  • Regularly assess the patient and family psychosocial needs.

  • Regularly assess the pre-bereavement/anticipatory grief needs.

  • Implement the Plan of Care provisions to meet their needs, which include: social service goals for alleviating problems, supportive counseling, problem solving, community referrals, etc
     

  • Qualifications

  • Master’s Degree from a school of social work accredited by the Council on Social Work Education

  • Minimum of one year social work experience in a healthcare setting or social services agency

  • Current Social Worker license

  • Home Health experience a plus

  • Knowledge of and ability to obtain community resources (information and referral, state/federal financial programs and eligibility requirements, etc.).

  • Strong organizational and communication skills
